The music world is still reeling from the loss of Ozzy Osbourne, and now, the official cause of death has been revealed.

According to his death certificate, the legendary rocker died of a heart attack, with coronary artery disease and Parkinson’s disease listed as contributing factors. The document, obtained by the New York Times, also gave a fitting tribute to his legacy—his occupation is listed as “Songwriter, Performer and Rock Legend.”

Ozzy passed away on July 22 at the age of 76, surrounded by family in his Buckinghamshire mansion. His death came just weeks after a surprise final performance with his original Black Sabbath bandmates at a charity concert in Birmingham. Fans had no idea it would be his last time on stage.

In a heartwarming clip filmed days before his passing, Ozzy appeared in good spirits during a casual family breakfast. Wearing a black t-shirt and headphones, he scrolled through his iPad as his daughter, Kelly Osbourne, playfully told him to say hello to the camera. He smiled and gave a cheerful “Good morning!”—a final glimpse into the softer side of the heavy metal icon.

Ozzy was diagnosed with Parkinson’s disease in 2003 but continued to make music and perform for as long as he could. Even in his final days, the Prince of Darkness never lost his spark.

His death marks the end of an era, but his legacy as one of rock’s most electrifying and controversial figures will live on forever.
